Make an Appointment

Patients are seen by appointment—on the same day the appointment is made if possible. To schedule an appointment, please call (603) 929-2137 during regular business hours.

Please call in advance for office visits. Make follow-up appointments as you leave. We make every effort to stay on schedule although emergencies do arise. If we are seriously delayed, we attempt to notify patients beforehand.

New Patients

New patients are asked to arrive 15 minutes early to complete paperwork. You can download, print, and complete the forms prior to your appointment. Please bring the completed forms, a list of all medications you are currently taking, recent copies of x-rays, lab results and your insurance card to your appointment.

Billing & Insurance

Payment for services is expected at time of service unless prior arrangements have been made. Cash, checks, and credit cards are all accepted.

Self-pay patients: Payment is expected at time of service. Discounts for payment in full are available.

Please bring your insurance card and I.D. with you to every visit so we can ensure that our records are accurate.

We recommend that you call your insurance company to verify that we participate with your plan.

Please notify us if you have any change in your name, phone number, address, or insurance as soon as it occurs. And please be prepared to present your insurance card at every visit.

Cancellations and Reschedules

To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments. We make every effort to stay on schedule, although emergencies arise.

Pharmacy/Prescription Services

Prescription medications are an important aspect of any patient’s care. We ask that you carefully follow all instructions, question anything that you don’t understand, and bring either your medications or a list of your current medications to each and every office visit so that you can review them with your provider.

Refill Policy

New prescriptions and medication changes must be taken care of at the time of your appointment. For prescription refills, please call your pharmacy directly. The pharmacy will contact our office if additional information is needed.

Lab and X-Ray Services

While some services are available on site, others may require a separate visit to the hospital or lab. Our office will provide instructions and directions to you when you are in need of these services.

Referrals to outside services must be reviewed and approved by our physicians before being processed.

Test Results

Your provider may order blood tests or other studies to be completed after your office visit. If anything is abnormal, your provider will call you right away to discuss the results. Normal results will be sent to you by mail—which may take up to two weeks from the date of your test.

Accessing Medical Records

Release of Records to a Specialist

If you need to have your medical record sent to a Specialist for an upcoming appointment, please call our office at (603) 929-2137 and have your name, your date of birth, the specialist name, the date of your appointment and the phone or fax number of the specialist. We will ensure that your records are faxed to the specialist’s office within 24 hours.

For a Complete Copy of Your Medical Record

If you need a copy of your entire medical record for a transfer to a new PCP, life or other insurance request, Workman’s Compensation, or for any other reason, you must sign a Release of Medical Record Information and fax or mail it to our office. Please allow up to two weeks for your request to be processed. There is an administrative fee for copying your medical record depending upon the number of pages that are copied. We must receive payment prior to sending out the copy. You will receive an invoice, if applicable, by mail, based upon your request.

Referral Services

For a referral to another medical office or specialty office, please call our office (603) 929-2137, and leave the following information:

  • Your name
  • Date of birth
  • Your phone number
  • Hampton Internal Medicine Provider name
  • The name of your Insurance company
  • The name of the specialist and when and why you will be seeing them

Please check with your insurance company to make sure your specialist is covered under your plan.

Please note—we do not make confirmation calls or send copies of the referral to anyone other than the specialist.

Referrals take 3 to 5 business days to process.

Please be aware that some out-of-network or out-of-state referrals can take up to 30 days to process.

Calling Your Provider

When our providers are seeing patients, it is often not possible for them to get the phone. Your provider (or the provider on call) will address non-urgent matters between patient visits, and an appropriate member of the team will return your call. Urgent matters will, of course, be handled as quickly as possible.

Phone System

You may reach us from 9:00am - 5:00pm at (603) 929-2137. After hours and during lunch, our answering service will page the covering provider in an emergency. Please use our regular office hours for questions about appointments, billing, prescription refills and referrals.

We realize that having a live person answer your call is the ideal situation. However, our staff members are often busy helping patients in the office and are unable to answer the phone immediately. In an effort to get you to the right person as quickly as possible, we now utilize an automated answering system to help direct your call. Please remember that you can hit “1”, “1” at any time to bypass the system.

Once the system answers your call, you can dial the following options:

  • Dial “1” if you are a hospital or physician
  • Dial “2” to schedule an appointment or speak to a Medical Office Specialist
  • Dial “3” to refill a prescription
  • Dial “4” to request an insurance referral
  • Dial “5” for billing related questions
  • Dial “6” for office information such as mailing address and fax number

After-Hours & Emergencies

If you are experiencing a medical emergency, dial 911. However, if you need after-hours advice for a medical concern, please call (603) 929-2137 and our answering service will assist you.
